Course content
History of social work in health care. Social work in the context of health care provision in the Czech Republic. Theoretical concept of social work in health care - holistic biopsychosocial perspective. Conditions for the performance of social work in healthcare. Professional profile Social worker in health care/health social worker. Social work in a medical facility/hospital with specific target groups: - patients in the gynecology and obstetrics department - children - seniors - patients with mental illness - patients in the terminal stage - patients with disabilities - patients - homeless persons - patients - foreigners - patients - members of other ethnic groups Continuity of health and social care upon discharge of patients from hospital to follow-up care. Long-term care provided in the healthcare system and in social services.

Learning outcomes
The course focuses on theoretical and practical aspects of social work in the context of health care. The aim of this subject is to provide students with the knowledge and skills needed for effective intervention in the field of health care, both at the individual, group and community level. Students will learn to effectively identify and respond to the needs of patients or their families in order to contribute to improving the quality of life of patients and their loved ones. The subject describes the efficiency and economy of the continuity of health and social care and gives examples of social and health services for selected target groups.
Student: - understands the importance of continuity of health and social care; - critically works with acquired knowledge of the health and social sectors; - can interconnect and look for connections in health and social care; - is capable/able from an administrative point of view to solve the social situation of the client; - can keep social-health records; - can use the gained knowledge in practice.

Assessment methods and criteria
Seminar Work

Recommended attendance at lectures. Processing and submission of the task on the topic: The role of the social worker in the care of a chronically ill patient. Credit will be awarded based on a positive assessment of the assigned task.
